Vision
Palliative Care at Trinity Village Care Centre enhances quality of life. It relieves pain and symptoms for the elder without expecting cure. This care includes the support of family, staff members, volunteers, and other elders. The acute palliative care phase is considered to be approximately six months of life remaining.
Palliative Care is a unique form of care that supports lived ones as they close this part of their life in anticipation of entering anew life. We who are invited to be a part of this journey, discover deeper meaning in the cycle of life within ourselves
Palliative Care Team. Saying good-bye becomes shalom (peace).
The Palliative Care team is an inter-disciplinary team consisting of members:
- Nursing
- Dietary
- Recreation

- Social Work
- Pharmacy
- Environmental
|
- Chaplaincy
- Volunteers
- Physician
|
This team works closely with the elder and the family to:
Deliver treatments needed to maintain comfort and support for the elder and family
Is accessible and maintains close contact with designated person from family
Makes all pertinent health information available to elder and family designate
Offers spiritual support and/or rituals through visits from the chaplain or other spiritual elder
Offers material on grief and dying
Introduces palliative care volunteers as needed or requested
Maintains an environment of rest and peace
Allows the elder to active as she/he desires
Informs elder/family of policy for funeral/memorial services here in chapel
Supportive therapy room is available to family wishing to stay at Trinity during the palliative period
